The Planarian Deadman (プラナリア・デッドマン, Puranaria Deddoman) is a planarian-themed Deadman created using the Planarian Proto Vistamp by Amahiko Haitani. Due to the Planarian Proto Vistamp's ability to replicate itself, a joint contract can be made between with an additional person. As a result, a second Planarian Deadman can be created. The copy Vistamp can be used by the second person to merge with either the primary contract holder's Deadman or their own to become the Planarian Deadman Phase 2 (プラナリア・デッドマンフェーズ2, Puranaria Deddoman Fēzu 2). Alternatively, the primary contract holder can use the original Vistamp to become the Phase 2 themselves in the conventional way.

The Planarian Deadman is the result of an inner demon being released from a human's body by the power of the Planarian Proto Vistamp. It has acquired the genetic information of a planarian, and even if it is given considerable damage, it can recover immediately due to its regenerative ability. It also has high fighting ability and entangles the target with its long tail, depriving them of freedom before attacking.

The Planarian Deadman and a person other than its host have been united by a joint contract that makes an advanced contract using a duplicated Vistamp. By repeating this, it is possible to mass-produce Phase 2 Deadmen from one Vistamp. By entering Phase 2, they have gained super-regenerative power in exchange for the risk of not being able to return to human form. Furthermore, it has been confirmed that when the original host makes an advanced contract, they become a powerful individual with more strength than the joint contract.

- Energy Slash: The Planarian Deadman Phase 2 can perform yellow energy slashes with Nodarake Arms.

- The Planarian Deadman is the first planarian-themed monster in the Kamen Rider franchise.
- The Planarian Deadman is the first Deadman who did not fuse with its contract holder to gain the Phase 2 upgrade.
- The Planarian Deadman has the most faces out of all Deadmen, having the same insect-like head all over its body.
- When the DX Juuga Driver revealed a refined version of the Proto Vistamp would be based off Kamen Rider Chaser, the nature of the Deadman would infer antithetical traits: A being with no fixed identity, abandoning their humanity for power, as opposed to Chase's pursuit of being more human, with his own identity separate from his template.
