Koji Maezono (前園 孝治,Maezono Kōji) is Hitoshi Maezono's father and the contract holder of Cheetah Deadman. He can transform into Cheetah Deadman Phase 2 (チーター・デッドマン フェーズ2,Chītā Deddoman Fēzu 2) by using the Cheetah Proto Vistamp.
Koji's son, Hitoshi Maezono, and three other members of the Ooshiromi Academy gymnast team were suspended due to stealing students' wallets. Hitoshi and two of the other boys then form a thief group. This results in Koji transforming into the Cheetah Deadman to distract the authorities so that the boys are not caught, as he is the director of a hospital and wants his son to get into medical school to follow in his footsteps. After seeing that the boys are wearing the jerseys of their high school, Ikki Igarashi learns their identities and comes to the conclusion that the Cheetah Deadman is also one of the four suspended students. Kamen Rider Revice battles the Cheetah Deadman during the next heist, but he is able to escape. When Ikki meets with Ooshiromi Academy's principal, he learns that one of the four suspended boys is overseas. He later overhears that Koji has been missing, leading Ikki to deduce that he is the Cheetah Deadman. When Hitoshi and the other boys attempt to turn themselves into the police out of guilt, Koji stops them but is confronted by Ikki. In the resulting battle with Kamen Rider Revice, the Cheetah Deadman is defeated and Koji reconciles with his son before the two are comforted by Ikki.

He uses his speed to break through anything that stands in his way. In addition, he has sharp claws and is good at tearing through targets as they pass by.

Enhanced Speed: The Cheetah Deadman Phase 2's most prominent ability is his immense speed, which allows him to outrun and outmaneuver almost any opponent as well as dodge numerous attacks.

The Cheetah Deadman Phase 2 is the second cheetah-themed monster after Cheetahkatatsumuri.
Chronologically, the Cheetah Deadman is the third Deadman to be Phase 2 by default, with the Chameleon Deadman being the first due to it already appearing since the events of Episode 1 and the Kong Deadman as the second.
