Kiyo-chan
Kiyo-chan (キヨちゃん, Kiyo chan) was a puppet which was regularly seen on the shoulder of Kiyoto Maki. It was originally given to him by his sister, Hitomi Maki as a means to cope with Hitomi's upcoming marriage.
The puppet originally had hair but it was burned after Kiyoto started a fire, something he had done to murder Hitomi. Since then, Kiyoto has used Kiyo-chan as a means to communicate with others, usually perching the doll upon his arm. Kiyoto is shown to cherish the doll immensely, outfitting it with different wardrobes and accesories. Whenever Kiyo-chan is mishandled, Kiyoto will panic and be unable to speak clearly, especially if the offender uses fire to damage Kiyo-chan.
Since Maki absorbed the purple Medals, Kiyo-chan has been shown in increasingly jarring poses and actions, even once channeling the same Greeed powers as Maki himself, implying some level of sentience. Possibly as a means for someone else to remember him if his plans failed, Kiyoto ultimately entrusted Kiyo-chan to Chiyoko before the final battle, dressing the doll in the same outfit as himself.
With the world ultimately saved by OOO and the others, Kiyo-chan was last seen Chiyoko's shop, being used as part of the interior for her restaurant.

OOO-era Movies[]
These movies, which were shown during the run of OOO, take place within a version of the TV series' timeline. However, there are some contradictions between story elements, making reconciliation with the show's continuity difficult.
Movie War Core[]
In Kamen Rider OOO: Nobunaga's Desire, Doctor Maki oversees the resurrection of Nobunaga in an operation comissioned by Kousei Kougami. He is unnerved when Kiyo-chan is dropped on the floor in the struggle to restrain the reincarnated Nobunaga.
OOO Wonderful[]
When the Greeed lend their Core Medals to OOO, Kiyo-chan us seen holding one of Maki's purple Cores attached to Kiyo-chan which joins with the remaining two in Eiji's body, allowing OOO to destroy Gara with the combined power of all of his Combos. Kamen Rider Fourze[]
Likely due to Kiyoto Maki's actor playing Yuki's father, Kiyo-chan makes a secret guest appearance in the series. To note, Kiyo-chan can be seen standing behind a white statue that place on Yuki's house gate [Light and Dark Twins] and seen again laying behind another statue that place near Dark Yuki.
