The Super (Upgrade) Form (超強化フォーム, Chō Kyōka Fōmu) is a recurring theme of form change for each Kamen Rider. It is stronger than the Upgrade Form and is usually the last form before a Final Form.
This is a type of form found in mostly Heisei-era Riders, though some such as Ryuki, Kabuto, and Decade did not have a Super Form before their Final Form.
More recent series, starting with Build, have introduced the concept of Super forms being a two-stage process, being only partially complete and needing to evolve further with an item to improve its effectiveness due to the negative drawbacks of its initial state. These drawbacks include the Rider System physically endangering its users or their mental state being affected by the power (initially making them Berserk forms), which originally in previous series necessitated the transition to evolve to the Final form.

- The main Riders' Super Forms can be generally grouped into five types.
- Agile (Faiz Axel Form, Blade Jack Form, Double FangJoker, OOO Tajadol Combo, Drive Type Formula, Build RabbitRabbit, Revice, Geats Boost Form MarkII): The user becomes more agile when gaining significant speed or flying ability.
- Bulky (Agito Burning Form, Fourze Magnetstates, Gaim Kachidoki Arms, Ex-Aid Maximum Gamer Level 99, Build TankTank, Iron Gotchard): The user gains more armor or muscle, increasing either his physical capacity or defensive power.
- Fusion (Den-O Climax Form, Kiva DoGaBaKi Form, Wizard All Dragon, Ghost Grateful Damashii, Zi-OTrinity, (Jack) Revice, Geats LaserBoost Form): The user combines his previous forms into one single power, allowing him to use all of these forms' capacity.
- Upgrade (Kuuga Amazing Mighty, Hibiki Kurenai, Build Hazard Forms, Zi-O II, Zero-One MetalCluster Hopper, Saber Elemental Primitive Dragon, Jack Revice, Platina Gotchard): The user increases his base form's Ability Parameters, resulting in the colors of said form changing to match its enhancements.
- Berserker (Agito Burning Form, Double FangJoker, Build Hazard Forms, Zero-One MetalCluster Hopper, Saber Primitive Dragon): Even though the user is granted a boost in overall power, something in the trinket causes them to lose control over themselves and attack friend and foe alike.
- Some Super Forms are strong enough to rival or even sometimes surpass the Final Forms in terms of raw power. However, the Final Forms still have abilities that enable them to surpass the Super Form in areas they can't; Gaim Kachidoki Arms, Drive Type Formula, and Ghost Grateful Damashii being the only examples of this so far.
- Cross-Z is the only Kamen Rider so far to have a super form that appears after their final forms chronologically. Coincidentally, the form is a recolored version of his primary form.
- Many Super Forms contain restrictions, side-effects and/or dangers when used. These problems are usually stabilized either due to some changes or when the Riders upgrade to the Final Form:
- Kamen Rider Faiz Axel Form: The user can only move at the speed of sound for 10 seconds upon activation before its Deformation revert him to normal.
- Kamen Rider Kiva DoGaBaKi Form: The user can only use this form for 5 minutes before he loses sanity.
- Double FangJoker: The users could lose control of themselves. However, Philip finally managed to control of the form with the help of Shotaro.
- Kamen Rider OOO Tajadol Combo: The user is more fatigued than when using a standard full combo. This is seen when a Greeed manages to defeat him, where Eiji is injured all over his body. Putotyra Combo does not feature this issue.
- Kamen Rider Wizard All Dragon: The user's mana is drained very quickly, not allowing the transformation to last. Infinity Style counters this issue by having infinite mana after Haruto intentionally revived Wizardragon by the creation of the Infinity Wizard Ring through the cyan Magic Stone (which are transmuted from his tears).
- Kamen Rider Drive Type Formula: The user is weakened by G-Forces when using a physical Full Throttle finisher. Krim states that Drive's body may burst upon using too many finisher power enhancements at once. Type Tridoron does not feature this issue.
- Kamen Rider Ghost Grateful Damashii: The user is paralyzed for quite a while when any of the Gammaizers activate or when some of the 15 Damashii are having an argument with each other. Mugen Damashii does not feature this issue.
- Kamen Rider Brave Fantasy Gamer Level 50: As a result of the demon king theme of Taddle Fantasy, the user can lose control and can go wild during a battle. Legacy Gamer Level 100 does not feature this issue.
- Kamen Rider Genm Zombie Gamer Level X: As a result of the "zombie" theme that dictates this form, the user frequently exhibits bizarre, lumbering, and awkward mannerisms. This would be fixed with Zombie Gamer Level X-0 using Kamen Rider Build Gashat's power.
- Hazard Forms (Kamen Rider Build): The user loses full control of himself after a short period of time from transformation. This is due to the form overstimulating specific parts of the brain and the body to fight at increased levels of strength, eventually causing them to lose conscious thought and turning the user into a relentless killing machine. Remedied with the development of the FullFull RabbitTank Bottle, which suppresses the negative side effects while increasing power as RabbitRabbit or TankTank. Genius Form does not feature this issue.
- Kamen Rider Zi-O II: The ability to foresee an enemy's attack in the future can be exploited by those who can move faster than what the Rider’s eye can track (i.e., GeizRevive, ZECT Riders with Clock Up).
- Zi-OTrinity: When Sougo, Geiz, and Woz argue amongst themselves, they are left vulnerable to an enemy's attack due to each controlling a portion of Zi-OTrinity’s body. Sougo's regal iron fist could sometimes put an end to the arguments.
- Zero-One MetalCluster Hopper: Due to the Imprisonment Chain malware within the the key created from the Ark's Three Dimensional Printing System, Zero-One MetalCluster Hopper causes the user to go uncontrollably wild and will attack anything in reach. Inside the suit, the user would remain aware of their actions and would try to gain control, but due to Ark's malice he would be unable to act on his will. This issue was solved after the malware was erased by the Progrise Hopper Blade, turning MetalCluster Hopper's advantages against Thouser.
- Saber Primitive Dragon: Because of the book's nature, said form results in the user's mind being suppressed by the soul of a vengeful and lonely dragon, causing them to act like a feral animal, attacking both friend and foe and using weapons unconventionally. This issue was solved after obtaining the Elemental Dragon Wonder Ride Book.
- Jack Revice: Detransformation causes Vice to manifest instead of Ikki, due to Vice switching places with Ikki, endangering both existences due to Ikki's nature as a human. Despite the switch having been undone, Ikki and Vice would share the same body, causing chaos, even making Vice wild due to Giff's imminent revival. The Thundergale Vistamp would put an end to all of this, despite it also carrying the risk of erasing their lives. Ultimate Revice does not feature this issue.
- Holy Live: The form itself sealed away Kagero, causing Daiji's moral compass to be thrown out of whack. He would gradually become more brash and reckless with each defeat he suffered. The return of Kagero and EvilytyLive's debut would make Daiji accept Kagero as his ally, thus disabling every issue so far.
- Geats Boost MarkII: Usage is significantly demanding on Ace's stamina due to the form's super speed and strength taxing his body. After the transformation is cancelled, he will faint from exhaustion. LaserBoost Form rectifies this issue by providing energy via the Laser Raise Riser.
- Iron Gotchard: The usage of mixture transmutation is a forbidden technique that combines a human with a single Chemy, and Iron Gotchard's endless consumption of passion and hope as alchemical ingredients places immeasurable burden on Houtaro's body, making him unable to move his body during battles and even be forced out of his transformations multiple times. Once this form was later upgraded with Crosshopper into Platina Gotchard, it no longer has these side effects.
