Entry Raise Form (エントリーレイズフォーム, Entorī Reizu Fōmu), or simply Entry Form (エントリーフォーム, Entorī Fōmu), is the base suit assumed by the players who participate in the Desire Grand Prix.[1] The players automatically transform into Entry Raise Form at the start of each round if they do not possess any Raise Buckles.
Components[]
A typical Entry Raise Form is composed of the following parts:
- Rider Head (ライダーヘッド, Raidā Heddo):[2] The head.
- Eye (アイ, Ai): The eyes. The visual information obtained here is transmitted via an internal display, protecting the player's eyes from intense light such as explosions that suddenly occur during a game.
- Crest Gear (クレストギア, Kuresuto Gia): The head-mounted device. It expands the player's head functions and protects the head with lightweight yet extremely strong composite armor. With a shape that symbolizes the Rider's crest on the ID Core, it also functions as a visual aid to increase the Rider's visibility.
- Audit Signal (オーディットシグナル, Ōditto Shigunaru): The inspection device. By constantly monitoring the Kamen Rider's condition, it diagnoses the continuity of transformation based on operating efficiency, damage, etc., and cancels the transformation if necessary.
- Blank Crusher (ブランククラッシャー, Buranku Kurasshā): The mouth section. The voice chat function enables clear conversations even with loud noise, allowing communication between Kamen Riders during cooperative play, etc.
- Desire Basic (デザイアベーシック, Dezaia Bēshikku): The suit-type gaming device. Using energy supplied from the Desire Driver, it raises the output of the player's physical abilities.
- Progressive Arm (プログレッシブアーム, Puroguresshibu Āmu): Entry Form's arms. They produce enough power to defeat the Jyamato by increasing the player's arm strength. Furthermore, various functions can be added by attaching expansion equipment.
- Entry Glove (エントリーグローブ, Entorī Gurōbu): Entry Form's gloves. They can be configured for high sensitivity to perform more delicate work than can be done with bare hands. They can also be customized with the features of various Raise Buckles.
- Progressive Leg (プログレッシブレッグ, Puroguresshibu Reggu): Entry Form's legs. They increase the player's leg strength to raise the game's survival rate. Furthermore, various functions can be added by attaching expansion equipment.
- Entry Boots (エントリーブーツ, Entorī Būtsu): Entry Form's boots. They boast excellent jumping power and running performance on the game field. They can also be customized with the features of various Raise Buckles.

- While all DGP Player Riders have the same suit for their Entry Raise Form, only six riders have signature accessories added, which sometimes boosts specific abilities.
- Geats is equipped with the Geats Tail (ギーツテイル, Gītsu Teiru).
- Tycoon is equipped with the Tycoon Bandage (タイクーンバンデージ, Taikūn Bandēji), which increases Tycoon's luck.
- Na-Go is equipped with the Na-Go Bangle (ナーゴバングル, Nāgo Banguru), which has the ability to increase the striking power of Na-Go's punches and kicks.
- Buffa is equipped with the Buffa Muleta (バッファムレータ, Baffa Murēta), which increases Buffa's fighting spirit and winning rate.
- PunkJack is equipped with the PunkJack Cloak (パンクジャックマント, Panku Jakku Manto), which increases PunkJack's jumping power.
- Hakubi is equipped with the Hakubi Purity (ハクビピュアリティ, Hakubi Pyuariti), which increases Hakubi's sixth sense.
- In Kamen Rider Geats: Transformation Lessons, it is revealed to transform into Entry Raise Forms, they don't need to shout "Henshin".
