The Desire Royale (デザイアロワイヤル,Dezaia Rowaiyaru) is a reformatted version of the Desire Grand Prix where participating Kamen Riders fight each other within a "last man standing" ruleset.
The Desire Royale is said to be the "worst game ever" that was reformatted from the Desire Grand Prix by Colus, who hijacked the tournament after seizing Girori's Vision Driver. There were three missions and regarding the rules, there are some sections that are different from the Desire Grand Prix's rules. The Baby Devil, the demon of Koshiro Igarashi, holds the key to the game.
Months after the aforementioned tournament's end, the Desire Royale itself would be restarted by Suel, with the players fighting to have their wish to be granted by the Goddess of Genesis. The winner not only can have their wish granted, but can also determine the fate of the Desire Grand Prix, whether to have it continue or end permanently. Unlike the previous Desire Royale, this incarnation consists of only one game.

Results: Tycoon, Buffa, and Na-Go are eliminated after Kamen Riders Knight, Ouja, and Ryuga interfere. Additionally, Live and Jeanne are also "killed" by the Jyamar Area alongside Tycoon and Na-Go respectively. Geats, Revi, Vice, and with the Mirror World Riders are permitted to progress to the next round, with Seeker joining after his arrival near the end of the first round.
Second Round
Game: Kamen Rider Extermination Game (仮面ライダー絶滅ゲーム,Kamen Raidā Zetsumetsu Gēmu)
Results: The round immediately transitions to the final round, and the eliminated players (as well as Live and Jeanne) return to battle Seeker and the Mirror World Riders.
Results: Seeker is defeated from battle against Geats, and Revi is able to reclaim Baby Devil. Ace and Ikki are declared winners to the Desire Royale, though Vice is excluded due to being a demon. Ikki declines his offer to grant his wish, so Ace grants his wish to have Ikki keep his memories of Vice.

The logo design is likely influenced on the Battle Royale logo, a film thought to be the origin of survival genre in media and also distributed by Toei Company.
