2019: Apocalypse (2019:アポカリプス, Nisen-jūkyū: Apokaripusu) is the forty-ninth and final episode of Kamen Rider Zi-O. It is the third and final part of the Kamen Rider Decade tribute arc, with the episode being a tribute to the entire Heisei era itself. It is also the final part of Zi-O's three-episode endgame arc. It features Sougo's transformation into Ohma Zi-O, the revivals of both Heure and Ora, having died in episodes 45 and 46 respectively.
Synopsis[]
Behold! The Rider of the Future, Kamen Rider Tsukuyomi, is born! However, this new power will be used in an unexpected way! Did Tsukasa and Sougo's plan ultimately fail?!
As the end of the world approaches, Sougo and Swartz must face each other one last time. Will the fate of the world be total salvation... or complete destruction?
Plot[]
Tsukuyomi has achieved success in becoming a Kamen Rider! But, as she moves forward, she stops time and kneels down to her brother, Swartz, pledging her loyalty. Having been seemingly betrayed, everyone else retreats as Swartz roars with laughter.
- "Tsukuyomi has finally become a Kamen Rider, in no small part thanks to the efforts of Sougo Tokiwa. Her choice to defect, however, was completely unforseen. This is the final page... until the end of the world..."

Sougo, Geiz, Woz and Tsukasa Kadoya return to 95 DO to discuss the recent situation. Tsukasa's original plan was to make Tsukuyomi a Kamen Rider because the cause of her world's destruction is the absence of a Kamen Rider there. Ideally, once Tsukuyomi becomes a Kamen Rider, she would return to her homeworld and then create a link with Sougo in his world, uniting both. Still, this plan lacks the means to migrate billions of beings on Zi-O's world to Tsukuyomi's homeworld.
Meanwhile, in the city, Tsukuyomi tells her brother about Tsukasa's plan. Swartz agrees with the plan, especially after Tsukuyomi promises to support Swartz's rule as the king in the new world.
The following morning, Sougo has breakfast with Junichiro. The uncle tells him not to make this their last meal together, and that he should come home with his friends when everything is resolved. Before Sougo departs for the final battle, Junichiro presents him with the repaired 18 Heisei Ridewatches, citing that for a master horologist like him, there is no watch he cannot repair.
In the city, the horde of innumerable villianous beings are wreaking havoc everywhere. Geiz, Woz, Tsukasa, and Daiki continue fighting to protect the civilians, but the enemies are resurrected endlessly, overwhelming the four Riders. The situation becomes even worse when Swartz summons some of the strongest villians of Riders. N·Daguva·Zeba brings down the absolute darkness from Kurogatake Mountains, Kamen Rider Evol consumes people inside his black hole, Gamedeus begins infecting people with the Gamedeus Virus, and the Sagittarius Zodiarts and Utopia Dopant slaughter civilians.
With the help of Tsukuyomi, Another Decade defeats the four Riders, taking them out of the battle. Before he could go on, Sougo arrives with the newly-restored GrandZi-O Ridewatch and summons Kamen Rider Build TankTank Form, Drive Type Formula, Ghost Ore Damashii, and Ex-Aid Maximum Gamer to assault Another Decade, disappearing as they finished their attack. Sougo then proclaims that Swartz can destroy history all he wants, but the Kamen Riders will exist forever. After that, Sougo transforms into GrandZi-O and summons OOO Tajadol Combo, Gaim Pine Arms, Wizard Infinity Style, and Double CycloneJoker Xtreme to aid him in battle, but the final bosses wipe the floor with them. Finally, with the combined strength of the final villains, Swartz defeats Sougo too, reverting him to human form.
Swartz targets Sougo and, with murderous intentions, blasts him; however, Geiz, in his GeizRevive Shippu armor, comes in at the last second and absorbs the brunt of the impact, negating his transformation and suffering severe injuries. Sougo rushes over to the dying Geiz and cradles him in his arms. Geiz takes the opportunity to finally confess to Sougo that he feels very happy that he came to 2018 and befriended Sougo. In his death throes, Geiz calls Sougo by his name one final time and tells him to become Ohma Zi-O, the strongest and kindest king he can become, before he ultimately succumbs to his wounds. Stricken with grief and anger, Sougo's Ziku Driver transforms into the Ohma Zi-O Driver.
An extremely angered Sougo heeds Geiz's final wish and, in an act that renders even Black Woz speechless, uses the Ohma Zi-O Driver to become the Demonic King of Time, Ohma Zi-O. In a low growl, Sougo commands Black Woz to announce his ascension, which the prophet does without hesitation. Swartz is extremely pleased to see the rise of Ohma Zi-O, and laughs in elation as he finally realizes that he can absorb his power and rule all 20 Heisei Rider worlds. Swartz begins to absorb said power; however, it backfires spectacularly as he is pushed back. Sougo then tells Swartz that someone like him is unworthy of Ohma Zi-O's power because it is the power of ALL Kamen Riders. Swartz doubles in pain as he commands his villainous minions to attack.
In the most one-sided battle of his entire life, Sougo uses his newfound power as Ohma Zi-O to dispatch all of the final bosses in one hit, even thinning out the horde of Riders' enemies effortlessly in the process. Seeing this, Swartz desperately tries to retreat back to his world and use what little of Ohma Zi-O's power he has absorbed to rule. However, just before the veil could pass over him, Tsukuyomi shockingly betrays her brother and stabs him from behind, declaring that their world is not worthy of a king like him. Swartz becomes enraged and easily kills Tsukuyomi after hearing this. Finally seeing that Swartz has killed everyone he holds dear to him, an even further incensed Sougo howls in anger before executing the Shuen no Toki: Ohma Zi-O Hissatsu Geki. Swartz prepares to defend Sougo's advance, but as Sougo's finisher comes down on him, the immense power and strength being displaced by the attack is far greater than even he can withstand. Swartz is eventually knocked back and screams in agony as he explodes in a massive fireball, destroying him, his legions of drones and the Another Decade Ridewatch with him, finally ending his tyrannical dominion over the 20 Heisei Rider Worlds once and for all.
In the future, Swartz is disintegrating before Tsukuyomi, who tells him to let go of all of his foolish actions and ultimately admit that no matter what he does, he can never defeat Ohma Zi-O, the strongest and kindest King of Time. Swartz once again tells Tsukuyomi that he still doesn't care about her opinion on the matter, before he is finally erased from all existences. Before she disappears, Tsukuyomi makes one final request to Sougo: she is entrusting to him the future of her world as well as his.
Black Woz, who was watching the battle from afar, congratulates Sougo on saving the world before pledging his everlasting loyalty to the newly crowned King of Time. However, in a shocking turn of events, Sougo tells Black Woz that he doesn't want to rule and that he is intent on destroying this timeline. Before he can proceed, Sougo is brought before his past self, who asks him if he is willing to discard his dream of becoming a king to create a new timeline. The present day Sougo tells him that to him, ruling no longer matters if all of his friends are dead. Hearing this, the past Sougo finally vanishes, happy to see that his mistakes will be resolved.
As Sougo begins to destroy the timeline, Tsukasa and Daiki watch from a distance. Tsukasa tells Daiki that his time in the World of Zi-O is finally nearing its end and that he looks forward to another great adventure in the future. Soon after, the united worlds finally separate back into the 19 Rider worlds as well as the World of Swartz, wholly restoring the timeline to its normal state.
In this new, unaltered timeline, Sougo returns as a high school student at the end of the 2018 summer vacation. As he departs, he meets up with Geiz and Alpina, who have become his school mates. Ora and Heure are also present, having become Sougo's underclassmen. The five friends then depart for school, surely to lead ordinary lives. The episode, season, and Heisei Rider era conclude with Black Woz, who was saved from the reset, observing from afar and writing down:
- "Ordinary high school student Sougo Tokiwa is destined for a future where he becomes Ohma Zi-O: the Demonic King of Time... Or is he? We may never know for certain. From this day forward, a new book, different from this one, will need to be written..."

- In the credits, Masahiro Inoue and Kimito Totani do not have the names, Kamen Rider Decade and Kamen Rider Diend next to their characters names even though they were transformed in this episode.
- When Decade inserted the Final AttackRide Card to NeoDecadriver, the text displays "AttackRide: Slash" on the buckle, while it announces "Final AttackRide: De-De-De-Decade!".
- In the GrandZi-O transformation sequence since the thirty-ninth episode, the CGI pedestals once again are placed perspectively wrong along with visible green screen lines.
- In addition, when Sougo rotates his belt, the clock's hand stop at the wrong spots.
- The GrandZi-O Ridewatch was reformed even though the Decade Ridewatch isn't in Sougo's possession anymore and was returned to Tsukasa in episode 47.
- Although, GrandZi-O did fight simultaneously with Decade against Another Zi-O II in the same episode as well. This may indicate that the GrandZi-O Watch is usable as long as the original Ridewatch or power stays intact.

Geiz's broken helmet from this episode
- GeizRevive Shippu's broken helmet, revealing part of Geiz's face is reminiscent of several other scenes featuring broken helmets throughout Heisei Rider series.
- After Ohma Zi-O's transformation, all the Heisei Era Ridewatches (including the ones that did not make any appearance previously) make a cameo appearance as they fuse into him.
- However, it wasn't stated if the occurrence of those Ridewatches meant that Rider powers from their owners were stolen in this process or not.
- It is not explicitly revealed nor hinted if Kamen Rider Aqua was revived by Sougo's timeline reset and if revived, remembers the original timeline or not.
- It is not clarified if half of the Decade's powers absorbed by Swartz were returned to Tsukasa (or even Ohma Zi-O) following Swartz's defeat or were destroyed along with Swartz.
- Although in Kamen Rider Zi-O NEXT TIME: Geiz, Majesty, Daiki still had minimal powers despite Another Diend had absorbed the majority of his powers, which completely restored after his defeat, possibly meaning that the other half of Tsukasa's powers were likely restored back to him.
- This episode is the second episode after Build's finale to not feature characters from upcoming series.
- Sougo screaming in rage is similar to how Emu screams in rage before punching Cronus.

- However, unlike Build RabbitDragon (being an Extra Final Form), Ohma Zi-O is Zi-O's True Final Form (真実の最終形態, Shinjitsu no Saishu Keitai), due to being one and same is because of Sougo's dystopian future self. Despite this, it is (in some cases throughout the media) still "erroneously" considered as such due to fitting the requirements for an Extra Final Form, whereas Grand Zi-O, even though it is a "minor Final Form", is still considered as Zi-O's "final form".
- Both the worlds are reset and only a few Riders remember the original world's events.
- The identities of the same Rider have a conversation in mind and one of them appears to fade away (Sento and Takumi for Build, current and initial Sougo for Zi-O).
- Instead of the successor Rider, a Ridewatch makes an appearance (albeit, this time, in the closing screen rather than the episode proper).

- Coincidentally, N·Daguva·Zeba is present in both battles.
- It is also a callback to Yusuke Onodera transforming into Ultimate Form in Kamen Rider × Kamen Rider W & Decade: Movie War 2010 to fight Decade Violent Emotion. Coincidentally, both of the heroes transform to defeat an opponent with Decade's powers.
- Furthermore, both of their colorations are similar. (Black body with Gold ornaments and Red visors).
